PWe thought posting some of the most common forms here would allow employees quick access without having to make a stop into the office. If you have any questions on how to complete these forms (or any others), please refer to the Employee Binder. If it remains unlcear afterwards, please contact your Supervisor who will gladly review it with you. These forms are now available in electronic format, and can be filled out and emailed to the appropriate person. A reminder that individual names are not to be emailed on any document. Initials can be used.
One of the most popular forms at Quest, incident reports are required any time something irregular occurs in a persons routine. Examples include: medication error, refusal of medication, safety concern, verbal or physical escalation, damage to property, going AWOL, etc. It is important to remember that incident reports must be completed during shift and submitted by the next working day. Serious incidents should be recorded in the same manner, however on-call or management should be notififed immediately.
Contact notes are most commonly used to track conversations or events between various members of the quest community or the public at large. Examples of when a contact note might be completed are: when you have concerns about someone, when you receive a complaint, when you want to tell your Supervisor something, when you want to praise a coworker, when you need an issue addressed, when an Individual requests for you to write one, etc.
Expense claims are required in order to claim mileage when transporting people throughout the agency. Please ensure that you submit your claims monthly. For more information regarding expense claims, please visit Policy GE-21.
With over 300 employees, timesheets help us ensure that you are paid accurately and in a timely manner. If you ever have discrepancies with your paycheque, please contact a member of the Employee Resource Centre. For further information on payroll, please visit Policy GE-10.
Some day to day documents.
Time off requests must be submitted to the Employee Resource Centre 7 business days prior to the first day you are requesting to have off. It the employees responsibility to ensure that the time has been approved, so please be sure to call a team member to confirm that they have found adequate staff to fill your shift.
During the peak seasons, please remember that the agency cannot grant all requests due to the high volume. It is important to submit requests early on to give the ERC plenty of time to look for your replacement.
An appointment record should be taken with you to any type of appointment you attend. This might mean a trip to the doctor or dentist, or maybe to court or to see a lawyer. Please fill out the form during the appointment and submit to the office immediately following. This assists the agency in maintaining up to date records and allows us to ensure we are meeting individual goals.
If you have a concern or a request for our maintenance department, please complete a work request. Your request will be reviewed by maintenance and any and all necessary work completed by the department.
Employee evaluations will be conducted annually. Evaluations will be completed by the staff member's immediate Supervisor and/or the Director of Services. Job descriptions will be reviewed with each evaluation.
It is important for all employees to keep the ERC updated with their current availability. There is high turnover in September with employees going back to school.
